치수에 염증 또는 괴사가 발생하면 보통 치근단 영역으로 확대되면서 치근단에 병소가 생성한다. 병소가 눈 아래쪽에 있는 제4전구치 뿌리에 생성하면 초기에는 염증성 농이 배출되지 못해 눈 밑이 부어오르고 농이 계속 축적되다 피부가 밖으로 터져서 나오게 되는 것이다.
